I earned a master’s degree in Community Counseling from the University of North Alabama in 2009. I have worked professionally in the counseling field since graduation, employed as a counselor by both Bradford Health Services and Riverbend Center for Mental Health. My credentials also include:

Contact your insurance company to verify your mental health benefits, asking whether licensed professional counselors are covered under your plan and whether or not you need a physician’s referral to counseling. When you call me for an appointment you will be asked about this coverage, and specifically your policy numbers and personal identifying information to include your co-pay and annual deductible amounts, and how much of the deductible you have met year-to-date.

You may have mental health benefits for yourself, and possibly your family, through an Employee Assistance Program (EAP) at work. The health program managing your EAP benefits will refer you to me after you contact them directly per your policy; your human resources office can help you access these benefits.

My fees are as follows (payment is due at time of service – I accept cash, check, Visa, and MasterCard):
$100 for initial intake, visit, and evaluation
$75 for a standard 50 minute individual session
$25 per group member for a standard 90 minute group session
$75 for a 50 minute session with more than one person
$150 for a substance abuse assessment. See note below.
A $25 charge may be incurred for failure to keep an appointment not cancelled within 24 hours of the scheduled appointment time.

Note: The substance abuse assessment fee is valid within the Shoals area (Quad cities). Additional expenses may be incurred if travel is required outside of the Shoals area to complete the assessment. An unlimited number of faxed assessments are included within a three month period of the completion of the assessment.
